Signature Protocols — Dr. Thiago Perfeito
Six structured treatment programs — each addressing distinct anatomical targets across face and body — built from Dr. Thiago Perfeito's clinical experience in regenerative medicine, energy-based technology, and injectable biostimulation.
Book ConsultationWhat Signature Protocols mean in clinical practice
Signature Protocols are commercial labels used to identify combinations of individually recognised procedures — injectables, biostimulators, energy-based devices — sequenced according to individual anatomical assessment. They do not constitute standalone medical methods registered with the CFM. Each component holds Anvisa approval and established clinical indication. Results vary by patient and are discussed at clinical assessment.
Signature Protocols are not a registered trademark, a brand name, or a marketing concept. They are clinical frameworks — structured, reproducible treatment programs derived from years of practice at the intersection of regenerative medicine, advanced injectables, and energy-based technology. The name is used here for clarity of communication, not as a proprietary claim.
What defines a Signature Protocol is the sequencing logic. Aesthetic medicine has evolved considerably in its understanding of facial and body ageing: the contemporary model is anatomical and layered — bone resorption, fat compartment descent, muscle hypertrophy or atrophy, dermal thinning, and collagen fragmentation do not happen independently. They happen simultaneously, in different proportions in each patient, and they interact. A treatment that addresses only one layer while ignoring the others produces a result that is partial at best and dissonant at worst — the filled lip on an unlifted face; the smoothed skin over an unaddressed structural deficit.
Signature Protocols are built on the premise that clinical outcomes improve when the treatment plan maps to the anatomical problem in its full complexity. This is not an argument for doing more to every patient — it is an argument for doing the right things in the right order. Some patients present with a single dominant concern that one modality addresses well. Others present with compound ageing across multiple layers that benefit from an integrated approach. The assessment determines which of these is true for each individual.
The anti-overfilling philosophy is central to how these protocols are designed. There is a persistent clinical phenomenon in which patients accumulate product across multiple sessions without a coherent plan — each visit adds volume to the same zone, and the result drifts from natural towards reconstructed. Signature Protocols are calibrated against this. Volume is used where structural deficit exists, not where the patient requests it. Collagen stimulation and tissue remodelling are preferred over mechanical filling when the underlying deficit is dermal rather than volumetric.
Each of the six protocols described on this page was built around a specific anatomical objective: full-face multi-layer rejuvenation; lower-face structural definition; cervical refinement; abdominal contouring; body contouring through regenerative fat grafting; and systemic skin and hair regeneration through exosome therapy. They can be delivered independently, in sequence, or in combination — depending on the clinical assessment and the patient's objectives.
Dr. Thiago Perfeito trained in regenerative medicine and aesthetic procedures at Harvard Medical School, Mayo Clinic, CHOP, and Esneca, and holds active membership in the American Society for Laser Medicine and Surgery (ASLMS), the American Academy of Anti-Aging Medicine (A4M), the Age Management Medicine Group (AMS), and the New York Academy of Sciences (NYAS). These protocols reflect a clinical perspective shaped by that training — grounded in evidence, cautious about overcorrection, and oriented toward long-term results rather than immediate-session maximalism.
The six Signature Protocols
Each protocol below is a clinical framework with a defined anatomical target, a sequenced combination of modalities, and a specific indication profile. Candidacy is confirmed at consultation — the descriptions below are summaries, not self-assessment tools.
1. Hybrid Face Lift
Target: Full-face multi-layer rejuvenation — structural, dermal, and neuromuscular — without surgery.
The Hybrid Face Lift addresses the four concurrent mechanisms of facial ageing in a single protocol plan: skeletal scaffold reduction (restored through periosteal-plane hyaluronic acid and biostimulatory fillers), fat compartment descent (redistributed through targeted volume replacement), dermal thinning and collagen loss (treated through Morpheus8 radiofrequency microneedling and HIFU energy delivery), and dynamic line formation (managed through anatomically precise neuromodulator placement). The result is a full-face improvement that does not read as a single procedure. Indicated for patients in their late thirties through sixties presenting with compound ageing across more than one layer. Typically delivered across two to three sessions over six to eight weeks.

2. Defined Jawline
Target: Lower-face structural definition — mandibular angle, chin projection, masseteric volume.
The Defined Jawline protocol uses botulinum toxin to reduce masseteric hypertrophy and reshape the lower-face silhouette, high-G-prime hyaluronic acid filler to restore periosteal projection at the mandibular angle and chin, and calcium hydroxylapatite biostimulator where dermal scaffolding at the pre-jowl or submalar zones is depleted. Indicated for patients with reduced mandibular definition, early jowling, masseteric widening, or chin under-projection. Protocol is designed for both men and women, with product volumes and placement vectors adapted to the anatomical target — angular definition for male anatomy, refined contour for female anatomy. Results typically maintained 12-18 months.

3. Elegant Neck
Target: Neck rejuvenation — cervicomental angle, platysmal bands, skin quality, décolleté.
The Elegant Neck addresses the neck's distinct anatomical challenges: thin skin with limited subcutaneous fat, active platysmal musculature, gravitational descent, and cumulative UV damage. The protocol combines neuromodulator injection for platysmal bands (Nefertiti lift technique), Ultraformer HIFU or Morpheus8 radiofrequency for skin tightening, biostimulatory filler for lateral neck and décolleté volume restoration, and targeted skincare prescription for photoageing management. Indicated for mild to moderate cervical laxity, platysmal banding, and skin quality deterioration — not for severe excess requiring surgical correction. Sessions are spaced four to six weeks apart; full results at three to four months.

4. Sculpted Core
Target: Abdominal contouring — skin tightening, subcutaneous remodelling, biostimulation.
The Sculpted Core combines radiofrequency microneedling (Morpheus8 body applicator), HIFU focused energy, and biostimulatory filler to address the abdominal region comprehensively: skin laxity from weight fluctuation, post-partum changes, or intrinsic ageing; subcutaneous tissue remodelling; and dermal quality improvement. The protocol does not replace surgical abdominoplasty in cases of significant skin excess, but delivers clinically meaningful results for patients with moderate laxity, skin texture concerns, and subcutaneous irregularity who prefer non-surgical management. Typically delivered across two to three body sessions spaced four to six weeks apart.

5. Lipocube
Target: Body contouring through regenerative fat grafting — volume, proportion, and tissue quality.
Lipocube is a regenerative fat grafting protocol combining autologous fat transfer with biostimulatory preparation to improve both the transferred fat survival rate and the receiving tissue quality. The technique uses the patient's own adipose tissue — harvested from donor sites with excess, processed, and reinjected into target areas — making it the only body contouring protocol here that uses the body's own biological material rather than synthetic or exogenous products. Indicated for body proportioning, gluteal enhancement, breast volume restoration, and facial fat grafting where the goal is natural-feeling volume with regenerative benefit. The protocol requires a minor surgical context for harvest — candidacy and procedural detail are confirmed at clinical assessment.

6. Exocube
Target: Skin and hair regeneration through MSC-derived exosome therapy.
Exocube uses mesenchymal stem cell-derived exosomes — nanoscale vesicles carrying growth factors, miRNA, and signalling proteins — to stimulate collagen synthesis, accelerate cellular repair, and reverse hair follicle miniaturisation. The protocol delivers exosome preparations via microneedling or direct scalp microinjection, depending on the target (skin quality or androgenetic alopecia). Unlike PRP, which relies on the patient's own platelet concentration and varies with individual platelet count and health, MSC-derived exosomes provide a standardised, high-potency regenerative signal. Indicated for patients seeking biological skin regeneration, hair density restoration, or adjuvant recovery following energy-based procedures. Typically two to four sessions spaced three to four weeks apart.
How protocols are designed, combined, and how to begin
Every Signature Protocol begins with a clinical assessment — not a menu selection. Patients do not arrive and choose a protocol from a list; they present with anatomical concerns and aesthetic objectives, and the assessment maps those to the appropriate framework, or a combination of frameworks, or a simpler single-modality plan if that is what the anatomy indicates. The six protocols described here cover the most common structural presentations encountered in practice. They are not exhaustive, and not every patient fits neatly into one of them.
Protocols can be combined when the clinical indication supports it. Common combinations in practice include the Hybrid Face Lift with Exocube — where the structural and volumetric work of the Hybrid is augmented by the regenerative skin quality improvement of the exosome protocol, particularly for patients with significant photoageing or skin thinning. Sculpted Core with Lipocube is another — where body contouring via energy-based remodelling is paired with regenerative fat grafting to address both laxity and volume in the same anatomical zone. The sequencing of combined protocols is designed to avoid interference between modalities; in most cases, a gap of four to six weeks between energy-based sessions and injectable sessions is maintained to allow tissue stabilisation.
International patients and Brasília-based diplomats and expatriates are seen regularly. Protocols that extend across multiple sessions are planned with flexible timing to accommodate travel schedules. For patients who cannot commit to sequential sessions, the assessment defines which single session delivers the highest clinical return, and a written maintenance plan is provided for continuity with a trusted clinician in the patient's home country. Teleconsultation is available for pre-visit planning and post-visit follow-up.
To begin, book a clinical consultation. The first appointment is an assessment only — photographs, anatomical mapping, discussion of objectives and constraints, and a protocol proposal. No procedure is performed at the first visit without explicit prior agreement. The consultation is available in English and Portuguese.

What are Signature Protocols?
Signature Protocols are structured, multi-modal treatment programs built from Dr. Thiago Perfeito's clinical experience in regenerative medicine, advanced injectables, and energy-based technology. They are not a branded product line — they are clinical frameworks that define which modalities are used, in what sequence, and for which anatomical target. The six current protocols cover full-face rejuvenation, lower-face contouring, neck rejuvenation, abdominal contouring, body contouring through fat grafting, and regenerative skin and hair therapy. Each is adapted to the individual patient after clinical assessment — they are not standardised menus applied uniformly.
-
Who designs the Signature Protocols?
Dr. Thiago Perfeito, CRM-DF 23199, a physician in regenerative and aesthetic medicine trained at Harvard Medical School, Mayo Clinic, CHOP, and Esneca. He holds active membership in the American Society for Laser Medicine and Surgery (ASLMS), the American Academy of Anti-Aging Medicine (A4M), the Age Management Medicine Group (AMS), and the New York Academy of Sciences (NYAS). The protocols reflect a clinical approach shaped by that training: evidence-based, anatomy-first, and conservative with volume — the goal is a result that reads as natural, not as a procedure that was performed.
-
How do I choose between them?
You do not choose between protocols before the consultation — the clinical assessment does. At your first appointment, Dr. Thiago Perfeito conducts a structured anatomical evaluation: photographs, mapping of bone, fat, muscle, and skin layers, and a discussion of your objectives and any relevant medical history. From that assessment, he identifies which protocol — or which combination of protocols — is appropriate for your anatomy and goals. Some patients present with a single clear indication; others benefit from a combined approach. Patients who arrive with a protocol pre-selected are welcome, but the proposal may change once the anatomy is mapped properly.
-
Are Signature Protocols offered to international patients?
Yes. International patients and Brasília-based diplomats and expatriates are seen regularly. Consultations are conducted in English and Portuguese. For patients who travel for treatment, protocols are planned around flexible session schedules when possible. Where a full multi-session protocol cannot be completed in a single visit, the assessment defines which single session delivers the highest clinical return, and a written maintenance plan is provided for continuity with a trusted clinician abroad. Teleconsultation is available for pre-visit planning and post-visit follow-up. The clinic is located in Lago Sul, 20 minutes from Brasília International Airport.
