Two of these deserve extra weight. The first is candidacy. A provider who has never told a patient that XERF is not the right tool is unlikely to be assessing carefully. The second is who does the work. Ask who will assess you and who will hold the handpiece, and expect a direct answer.

Why the device and the doctor are separate questions

Patients often search for the machine and assume the doctor follows. The published studies cannot settle which matters more, because none compares operators or devices. A prospective multicentre study published in Cureus in 2026 followed 39 patients who had two XERF sessions four weeks apart; 84.6% reached an investigator-rated aesthetic improvement score of 3 or higher at 30 days and 92.3% at 90 days, with no lasting adverse events. It had no control group, follow-up was three months, and the manufacturer supplied the device, the consumables and research funding. A retrospective study in Lasers in Medical Science in 2026 followed 16 women after a single session and reported a redistribution of volume, lifting of brows and upper eyelids, and better wrinkles and pores at up to three months; its authors call the findings preliminary, and the first author is an international trainer for the manufacturer.

A computational and pig-skin study, also in Lasers in Medical Science, in 2025, explains the design logic: the 2 MHz frequency heated deeper and wider in fatty tissue, while 6.78 MHz concentrated along the fibrous septa, and together they produced a thermal reaction at the junction between dermis and subcutaneous tissue. That study is not clinical, and its authors are linked to the manufacturer.

Taken together, these papers describe a technology with a coherent rationale and favourable early results. They do not show that XERF beats other radiofrequency devices, and none compares devices directly. That is why it is reasonable to choose a provider on judgement and transparency, not only on the name of the machine.

Questions to ask before your first session

These questions work at any clinic, and they are also the ones a careful physician expects. The answers should be specific, not general reassurance.

  1. Who will treat me, and what is their CRM? The name should be one you can find in the CFM physician search.
  2. Which device is it, and what is its Anvisa registration? A clinic that cannot name the manufacturer is a warning sign.
  3. Am I a good candidate? A useful answer mentions the degree of laxity, skin thickness and what XERF will not change.
  4. What would you recommend if XERF is not right for me? Volume replacement, a collagen biostimulator or surgery may be the honest answer.
  5. Do you have other radiofrequency devices? If yes, ask how the choice between them is made.
  6. How many sessions do you expect, and when do we judge the result? Response builds over weeks to months, so photographs taken right after a session understate it.
  7. What is the price for each area, and what does it include? Ask for a range in writing and for the number of areas covered.
  8. What are the risks and who do I call if something goes wrong? Redness and mild swelling are expected; you should also be told who to contact after the session.
  9. Do I have a pacemaker, another active electronic implant, or a metal implant in the area? Am I pregnant? These need to be declared, because they change or rule out treatment.

Who should and should not have XERF

Candidacy is where a provider's honesty shows most. The best-fitting patient has mild to moderate laxity, skin that still has thickness and elasticity to respond to heat, and wants firmness without surgery or time away from work. In the face this can include the jawline, the neck, and the area around the eyes, where small handpieces are used.

XERF does not restore lost volume and does not remove excess skin. When a face looks slack because fat compartments have thinned, the sensible route is volume or a collagen biostimulator; when there is genuine skin excess and descent, the answer moves toward surgical support.
