In the tirzepatide group, body weight fell by 21.3%, fat mass by 33.9% and lean mass by 10.9%; in both groups, roughly three quarters of the weight lost was fat and about a quarter was lean mass. These are whole-body figures, not measurements of the abdomen, arms or thighs, and the authors have links to the drug's manufacturer. What they show is that weight lost this way includes some lean mass as well as fat. The study did not measure skin.

In SURMOUNT-5, published in the New England Journal of Medicine in 2025 (751 participants without diabetes, 72 weeks, funded by the drug's manufacturer), tirzepatide produced a 20.2% weight reduction versus 13.7% with semaglutide, and waist circumference fell by 18.4 cm versus 13.0 cm. Those are trial averages for medication outcomes; the trial did not assess skin.

Abdomen

When surgery is the honest answer

Non-surgical treatment improves skin quality and, in mild to moderate cases, produces visible tightening. It does not remove tissue. If you have a fold of skin that hangs over the waistband, skin that drapes from the arm, or the sort of excess left by a very large weight loss, energy devices and injectables can improve texture and support but will not change the amount of skin present.
