Radiesse, manufactured by Merz Aesthetics, is approximately 30% CaHA microspheres carried in roughly 70% carboxymethylcellulose gel. It belongs to a single class: a pure collagen biostimulator. There is no hyaluronic acid in the formulation, and the gel carrier itself is not a filler in the dermal-filler sense — it is a delivery vehicle that reabsorbs over weeks, leaving the CaHA scaffold to do the biological work.
HarmonyCa, manufactured by Allergan Aesthetics/AbbVie, is a hybrid by design. The same active mineral, CaHA, is suspended not in carboxymethylcellulose but in a cross-linked hyaluronic acid gel, with lidocaine included for comfort, in a single prefilled syringe that needs no reconstitution. That HA component is what a plain CaHA product like Radiesse does not have — and it changes the immediate behaviour of the product on injection.
Getting this distinction right matters clinically, not just semantically. A patient who wants a pure, predictable biostimulation pathway with no hyaluronic acid involved is asking a different question than a patient who wants immediate contour correction alongside a bioactive stimulus. Confusing the two products because they share a mineral name overlooks the part of the formulation that actually determines what happens in the tissue in the first weeks after treatment.
What the hybrid adds — and what it costs you in predictability
The hyaluronic acid in HarmonyCa is not a minor addition. It is what gives the product its instant, visible lift on the day of treatment — something plain CaHA in a carboxymethylcellulose carrier does not deliver in the same way, since Radiesse's gel provides only modest immediate volume before it reabsorbs. The panel states its own reason for writing them: the hybrid is recent compared with other injectable products, so the published literature — clinical practice recommendations included — is limited, and the objective was to set out safe and effective practice for injecting it. Its technique advice is concrete rather than abstract: inject into the superficial subcutaneous layer, in small volumes, with boluses and large volumes not recommended, using a cannula rather than a needle, and defining entry points by reference to the line of ligaments and the zygomatic arch.
Radiesse, by contrast, offers one mechanism end to end. There is no HA fraction to account for, no separate reabsorption curve layered on top of the collagen response. What you inject is what continues, on a single trajectory, from carrier-gel volume toward CaHA-scaffolded collagen. For a patient or a physician who values predictability over immediacy, that single-mechanism simplicity is itself a clinical argument.

Neither answer is superior in the abstract — which one is right depends on the anatomy being treated, the degree of volume loss already present, and how much immediacy the patient actually needs versus how much they are willing to wait for. A patient with diffuse volume loss and no urgency for a same-day change is often better served by the single-mechanism predictability of a plain CaHA biostimulator. A patient who needs a visible correction on a specific date, alongside a longer-term stimulus, is the more natural fit for the hybrid design.
Where each one fits by anatomy and age
Case selection follows the same logic as the timeline question, applied to specific anatomy. These two products tend to be compared by someone weighing more than one thing at once — structural support on the one hand and skin quality on the other — and what settles the choice is the anatomy being treated together with how much of the result the patient needs to see on the day.
Radiesse tends to fit best when:
- The goal is mid-face or cheek volume restoration with a single, well-understood mechanism
- The treatment area is extra-facial — neck, décolletage, or the back of the hands — where the hyperdiluted technique addresses skin laxity and quality across a larger surface without adding bulk
- The patient prefers not to introduce a hyaluronic acid component into that particular treatment plan
- A single-session protocol is preferred where anatomy allows
HarmonyCa tends to fit best when:
- The patient wants visible correction and biostimulation addressed in the same appointment, without a separate HA filler session beforehand
- The treatment area is the cheek — the primary injection region in the published post-marketing study, with the jawline as an optional second area — or, more broadly, the lateral and less mobile areas of the face that the expert panel recommends, with entry points referenced to the line of ligaments and the zygomatic arch
- The patient is looking to reduce the total number of separate injection sessions in their plan
Contraindications common to both: active infection or inflammation at the injection site, known hypersensitivity to any product component, pregnancy and breastfeeding, and autoimmune conditions or immunosuppressive therapy pending clinical assessment. For the hybrid specifically, the expert panel adds skin diseases and abnormal skin conditions at the site, and advises against injecting it in the lips or the perioral, periocular and glabellar areas. That same panel positions surgery as an alternative rather than as a waiting period: severe facial aging that is better addressed surgically is a reason not to select the hybrid at all. Dilution changes the product more than the brand does
There is a third layer of confusion beyond the brand names: Radiesse itself is not one product, but two very different clinical tools depending on how it is diluted. Standard Radiesse, injected close to its manufactured concentration, behaves as a volumizing biostimulator — used where structural support and a scaffold effect are the goal. Hyperdiluted Radiesse, mixed with saline or a local anaesthetic solution at ratios well beyond the standard preparation, is a different clinical tool altogether: a skin-quality and skin-tightening treatment distributed across a much larger surface area, in a more superficial plane, to stimulate collagen and elastin formation over time — whereas it is undiluted CaHA that is used to provide volume.

In other words, the difference between "Radiesse for cheek volume" and "Radiesse for neck laxity" can be larger, clinically, than the difference between standard Radiesse and HarmonyCa in some indications. The brand name alone does not tell you which job the syringe is doing.
HarmonyCa does not have an equivalent dilution decision built into its use. A separate prospective, open-label, post-marketing study of the same hyaluronic acid and calcium hydroxyapatite hybrid treated adults rated Moderate to Severe on a midface volume deficit scale, injecting the cheek as the primary region with the jawline optional, and assessing them at Months 1, 3, 6, 9 and 12 — a structured protocol with entry criteria and scheduled visits. An Italian retrospective real-life study looked back at patients who had already received the hybrid filler, evaluating its effect on restoring connective tissue quality. A fourth source, a prospective study, used three-dimensional facial scanning on eight patients to quantify the hybrid's volume and lifting effect out to 90 days.
